血尿酸与尿微量白蛋白及肾功能关系的探讨

摘    要:目的探讨血尿酸与尿微量白蛋白及肾功能相关性。方法回顾性分析本院2009年1月到2012年10月内科门诊就诊的185例血尿酸升高患者的血尿酸水平与尿微量白蛋白、血肌酐、血尿素氮之间的关系,进行统计学处理,并与血尿酸正常人群进行对照。结果观察组尿微量白蛋白(73.51±54.87)mg/L、血肌酐(83.60±18.09)μmol/L、血尿素氮(5.31±1.92)mmol/L均显著高于对照组的尿微量白蛋白(18.02±13.61)mg/L、血肌酐(67.84±15.84)I,zmol/L、血尿素氮(4.67±1.74)mmol/L,差异均有统计学意义(P〈0.05);观察组尿微量白蛋白、血肌酐、血尿素氮均随病程延长而升高,差异均有统计学意义(P〈0.05);血尿酸水平升高与尿微量白蛋白、血肌酐、血尿素氮水平正相关(P分别为0.005、0.034、0.042)。结论血尿酸升高可导致肾功能损害,早期干预有助于防治高尿酸致。肾损害的进一步发展,减少尿毒症的发生。

Abstract:Objective To discuss the relationship among serum uric acid, urinary microalbumin and renal function. Methods 185 medical clinic patients from January 2009 to October 2012 with higher serum uric acid level were studied. Retrospectively analysis was conducted to observe the relationship between their respective serum uric acid and urinary microalbumin, serum creatinine and blood urea nitrogen, statistical analysis, and compare with normal uric acid group. Results The urinary microalbumin, serum creatinine and blood urea nitrogen of observation group were significantly higher than those of control group (P〈0.05), and significantly elevated as extension of the course (P〈0.05). The elevation of serum uric acid level was positively related with the level of urinary microalbumin, serum creatinine and blood urea nitrogen. Conclusion Elevated serum uric acid can cause renal impairruent. Early intervention would help to prevent from further development of renal damage caused by high uric acid and reduce the incidence of uremia.
