二甲双胍治疗青少年2型糖尿病的临床观察

目的评价二甲双胍治疗青少年2型糖尿病的疗效及安全性.方法采用二甲双胍单独治疗8岁以上青少年2型糖尿病38例,治疗12周,观察其疗效与安全性,并与13例采用非双胍类药物者进行比较.结果治疗后与治疗前比较,无论是治疗组或对照组的空腹血糖、餐后2h血糖及糖化血红蛋白(HbA1c)均有明显下降(P均＜0.05),治疗组体重及血清甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-ch)也有明显下降(P均＜0.05),且优于对照组(P均＜0.05).治疗组的患儿服药12周后仅有1例丙氨酸转氨酶(ALT)从治疗前的28 U/L上升至46 U/L,其余两组患儿的ALT、血总胆红素(BIL)、Cr、BUN、尿微量白蛋白排泄率(UAER)以及WBC、Hb与治疗前比较无明显变化(均P＞0.05).结论青少年2型糖尿病患儿服用二甲双胍治疗与成年人一样可明显地降低血糖和体重,改善异常血脂,且比较安全.

A study of metformin on adolescent patients with typ2 diabetes melliuts

Objective To evaluate the efficacy and safety of metformin in adolescent patients with type 2 diabetes mellitus. Methods This study compared metabolic changes in drug-naive type 2 diabetes adolescents received either metformin or other oral antidiabetic agents for 12 weeks. 38 patients received metformin and 13 patients were prescribed other oral antidiabetic agents. Results HbA1C, fasting and postprandial plasma glucose level decreased in both treatment groups. (P<0.05) Significantly greater reductions in weight, triglycerides and low density lipoprotein cholesterol were observed in the metformin group. (P<0.05) Serum ALT level of only one adolescent was elevated from 28U/L to 46U/L after 12 weeks of treatment with metformin. Other side effects were not found. Conclusions Metformin in adolescents with type 2 diabetes provides significant antihyperglycemic efficacy and play a role in weight reduction and managing lipoprotein abnormalities as in adults. Oral metformin is generally well tolerated and has mild side effects.
