高血压病患者左心房扩大及左心室结构和功能改变

摘    要:分析53例高血压病(EH)患者多普勒超声心电图,结果表明,EH左心房扩大并非罕见(15.1%),左心室结构改变43.4%,其中左心室扩大15.1%,左心室肥厚28.3%,单纯左室舒张功能障碍(LVDD)37.7%,左心室结构和功能均正常18.9%,左心房扩大发生率随左室结构改变而增高(P<0.05),EH左心室肥厚以前可有LVDD,且随左心室肥厚而加重(P<0.01),左心室扩大者射血分数(EF)明显减低(P<0.01)。提示EH患者左心室肥厚以前尚有其他因素影响左室舒张功能,LVDD是高血压性心脏病的早期改变,左心室扩大以前无明显收缩功能损害。

Abstract:Doppler ultrasound cardiogram of 53 patients with essential hypertension (EH) is analyzed in this paper. The results showed that EH left atrial dilatation was not uncommon (15.1%),structural changes of left ventricle showed 43.4%,among them left ventricular dilatation accounted for 15.1% ; left ventricular hypertrophy,28.3%; simple left ventricular diastolic dysfunction(LVDD),37.7%;structure and function of left ventricle were normal,18.9%;the incidence of left atrial dilatation increased along with the changes of structure of left ventricle( P <0.05). The LVDD might occur before EH left ventricular hypertrophy occurred and aggravated along with left ventricular hypertrophy ( P <0.01),the ejection fraction(EF) of patients with left ventricular dilatation markedly decreased ( P <0.01),suggesting that there had yet another factors influencing left ventricular diastolic function before left ventricular hypertrophy occurred. LVDD was the early change of EH heart disease without marked damage of systolic function before ventricular dilatation occurred.
