不同病变胆囊组织中P27kip1、CDK2、Cyclin E的表达及相关性研究

目的通过对胆囊结石、胆囊腺瘤性息肉及胆囊癌组织中细胞周期蛋白激酶抑制蛋白P27kip1、细胞周期蛋白依赖激酶CDK2、细胞周期素Cyclin E的表达及相关性研究,分析在3种胆囊病变组织中的表达与病理的关系,以探讨其与胆囊癌的发生、发展有关的可能因素.方法应用免疫组织化学的方法分析人类3组胆囊组织中的P27kip1、CDK2、Cyclin E的表达.结果P27kip1在胆囊癌的表达与CDK2的表达呈显著负相关(r=-0.949,P＜0.05);CDK2与Cyclin E的表达呈显著正相关(r=0.908,P＜0.01).胆囊癌中P27kip1的表达与Nevin病理分期呈显著负相关(r=-0.558,P＜0.01),而与患者的性别、年龄、分化及病理类型无明显关系(P＞0.05)P27kip1在胆囊癌与胆囊结石之间的表达差异有显著性(P＜0.01),胆囊结石与胆囊息肉之间的表达差异也有显著性(P＜0.01);CDK2、Cyclin E在胆囊癌与胆囊结石的表达差异有显著性(P＜0.01),胆囊结石与胆囊息肉之间的表达差异也有显著性(P＜0.05).结论胆囊癌中P27kip1、CDK2、Cyclin E基因出现异常表达,P27kip1的表达抑制及CDK2、Cyclin E的过度表达在胆囊癌的发生、发展过程中可能起促进作用.P27kip1、CDK2、Cyclin E并不能作为预测肿瘤发生,诊断早期肿瘤理想的指标;P27kip1的低表达是胆囊癌预后不良的预测指标之一,CDK2、Cyclin E作为单独的判断胆囊癌预后的指标尚缺乏足够的依据.

Expression of P27kip1, CDK2, Cyclin E in variant tissue of human gallbladder and relationship to each other

To investigate the relationship of the expression of P27kip1, CDK2, Cyclin E in cholecystolithiasis, polyps and carcinoma of gallbladder. Through the study of the relationship of its clinical and pathological significance, we maybe find some factors about the genesis and development of carcinoma of gallbladder. Expression of P27kip1, CDK2, Cyclin E in variant human gallbladder tissue was investigated by immuneohistochemistry technique in protein level. In carcinoma of gallbladder, a negative correlation was noted between expressions of P27kip1 and CDK2(r =-0.949, P <0.05), whereas a positive correlation was noted between the expressions of CDK2 and Cyclin-E (r =0.908, P <0.01). A negative correlation was noted between expressions of P27kip1 and pathological grading (r =-0.558, P <0.01). There were not associated with these factors, such as sex, age, stage and cellular grade of the carcinoma. The difference between carcinoma of gallbladder and cholecystolithiasis of P27kip1, CDK2, Cyclin E was prominent. It was as the same as cholecystolithiasis between polyps of gallbladder. Conclusion] The results suggest that the inhibition expression of antioncogene, P27kip1 and the over expression of CDK2, Cyclin-E might be promoted factors of carcinoma of gallbladder. P27kip1, CDK2, Cyclin E can not be a marker about the early diagnosis. The inhibition expression of P27kip1 is suggestive of prognostic marker.
