| 摘 要： | 目的 探讨抗中性粒细胞胞浆抗体(ANCA)在狼疮性肾炎(LN)中检测的临床意义.方法 采用间接免疫荧光法(IIF)和酶联免疫吸附试验(ELISA)检测79例LN患者中的ANCA,分成ANCA阳性和阴性组,分析所有患者的临床表现、并发症、免疫学检查结果,并分析ANCA与临床表现和其它实验室结果之问的关系.结果 用IIF法检测ANCA在LN中的阳性率3 1.6%(25/79),对IIF法检测阳性的血清,用ELISA进行验证,结果阳性率为26.6%(21/79).79例中ANCA阳性组均为核周型ANCA(P-ANCA),无胞浆型ANCA(C-ANCA).ANCA阳性组中浆膜炎(76.2%)、神经系统累及(28.6%)、急性肾衰(61.9%),明显高于ANCA阴性组(P＜0.05),同时ANCA阳性组在贫血(Hb≤11g/dL)、抗SM抗体、补体C3、抗ds-DNA方面差别有统计学意义(P＜0.05).结论 ANCA有助于判断LN患者的病情活动程度.
