环氧化酶-2在肺癌组织中表达的研究

摘    要:目的:探讨环氧化酶-2(COX-2)在肺癌组织中的表达水平及其与病理分型、分化、TNM分期及预后的关系.方法:选择125例肺癌组织石蜡切片,应用抗人COX-2抗体进行免疫组织化学染色,用COX-2阳性表达血管内皮细胞作阳性对照.结果:鳞癌COX-2表达阳性率59.18%,腺癌85.71%,大细胞癌41.18%,小细胞癌20.83%.肺腺癌COX-2阳性表达率明显高于其它三类肺癌;鳞癌COX-2阳性表达率明显高于小细胞癌.COX-2的表达与腺癌的分化程度、TNM分期有密切相关,与鳞癌分化程度无明显关系.与年龄、性别、是否吸烟无关.COX-2阳性患者5年生存率19.57%,中位生存期39个月,阴性患者5年生存率40.54%,中位生存期为55个月,两组差异有显著性意义.结论:COX-2在肺癌尤其是腺癌和鳞癌中具有较高的表达,在腺癌患者,COX-2表达与肿瘤分化程度密切相关,鳞癌患者与肿瘤的分化程度无关.肺癌患者的肺内COX-2表达与年龄、性别、是否吸烟无关.COX-2高表达的肺癌患者预后较差.

Abstract:Objective: To study on the relations among COX-2 expression in lung cancer, its different class of pathology, differentiation, TNM period and brognosis. Methods: Immunostaining was performed with COX-2 antibody to the 125 surgically resected tissue samples. Vessel epithelium cell COX-2 expression was taken as positive outcome. Results: The COX-2 positive rate was 59.18% in squamous cell carcinoma, 85.71% in adenocarcinoma, 41.18% in large cell lung cancer, 20.83% in small cell lung cancer. The COX-2 positive rate of adenoarcinoma was greater than the others ( P <0.01), the COX-2 positive rate of squamous cell carcinoma was greater than that of small cell lung cancer ( P <0.01). COX-2 expression was associated with the extent of adenocarcinoma differentiation and TNM period, but was indpendent of the extent of squamous cell carcinoma differentiation. COX-2 expression was independent of age and sex of patients. Smoking or not is independent of COX-2 expression. In the COX-2 negative group, the 5-year survival rate and a median survival time were 19.57% and 39 months. In the COX-2 positive group, the 5-year survival rate and a median survival time were 40.54% and 55 months ( P <0 05). Conclusion: COX-2 expression in lung cancer especially in quamous cell carcinoma and adenocarcinoma is greater than normal lung tissue. COX-2 expression was associated with the extent of adenocarcinoma differentiation and independent of the extent of squamous cell carcinoma differentiation, age, sex and smoking or not of patients. The prognosis of the lung cancer patients with COX-2 over-expression is worse.
