双手微切口白内障超声乳化术的超声能量应用研究

摘    要:目的研究双手微切口白内障超声乳化术的超声能量应用特点。方法将各级核硬度的年龄相关性白内障167例(170眼)随机均等分为两组,分别行双手微切口白内障超声乳化术和常规3.2mm透明角膜切口白内障超声乳化术,观察两组的术中平均超声能量、超声乳化时间、累积能量复合参数、术中并发症及术后早期效果。结果双手微切口白内障超声乳化术处理各级核硬度白内障所用的平均超声能量均显著低于常规术式(P<0.01);对于中等核硬度白内障,超声乳化时间和累积能量复合参数值皆显著低于常规术式组(P<0.01)。常规术式组中有2例发生切口灼伤,而双手微切口组未观察到此并发症。双手微切口组和常规术式组术后第1d裸眼视力不低于0.5者分别为63眼(74.12%)和52眼(61.18%),两组间差异无统计学意义(P=0.071)。结论双手微切口超声乳化术的超声能量应用效率较高,手术安全,疗效良好。

Abstract:ObjectiveTo evaluate the application features of ultrasonic power during the bimanual microphacoemulsification.Methods170 age-related cataract eyes of various degrees of nuclear hardness were comprised in this study.The subjects were equally and randomly divided into 2 groups,and the bimanual microphacoemulsification and conventional 3.2 mm clear corneal incision phacoemulsification was separately performed on the operative eyes in the two group.The study parameters including average power,ultrasonic phacoemulsification time,the accumulated energy complex parameter(AECP),uncorrected visual acuity on the 1st postoperative day,and the intraoperative complications were observed.ResultsThe average power used during bimanual microphacoemulsification was significantly lower than conventional phacoemulsification(P<0.01).The ultrasonic phacoemulsification time of bimanual microphacoemulsification was significantly shorter than that of conventional procedure,and so was the case with AECP(P<0.01) for the cataract with medium nuclear hardness.Incision burning occurred in 2 cases with hard nucleus in the conventional group.On the 1st postoperative day,the uncorrected visual acuity was better than 0.5 in 63 eyes (74.12%) in the bimanual microphacoemulsification group and 52 eyes(61.18%) in the conventional phacoemulsification group without statistically significant difference between them.ConclusionUltrasonic power application during the bimanual microphacoemulsification has an effective,save and satisfactory early postoperative outcome.
