综合护理对腰椎间盘突出症患者手术前后依从性影响

目的探讨综合护理措施对腰椎间盘突出症患者手术前后阶段性功能锻炼依从性的影响。方法选择2011年4月-2012年11月收治于安徽省宿州市中医医院的腰椎间盘突出症患者136例，以2011年4—12月68例患者为对照组，以2012年1--11月68例患者为实验组。对照组采用常规护理方法，实验组除常规护理外在不同阶段（术前和住院治疗阶段、出院康复阶段）按照阶段性功能锻炼模式实行规范化管理，比较两组患者功能锻炼依从性、术后并发症发生率和护理满意度。结果实验组患者功能锻炼依从性增强，住院治疗阶段完全依从60例，部分依从8例；对照组完全依从20例，部分依从42例，完全不依从6例，两组比较差异具有统计学意义（P〈0．05）。实验组没有下肢静脉血栓发生，对照组发生6例，两组比较差异具有统计学意义（P〈0．05）。结论阶段性功能锻炼措施不仅能有效提高腰椎间盘突出症患者手术后功能锻炼的依从性，且能降低患者术后并发症发生率，对患者的早日康复起到了积极的推动作用。

The influence of comprehensive nursing care on complianceto functional exercises in patients with lumbar intervertebral disk protrusion pre-and post-operation

Objective To investigate the influence of comprehensive nursing care on compliance to functional exercises in patients with lumbar intervertebral disk protrusion. Methods A total of 136 cases with lumbar intervertebral disk protrusion in our hospital were chosen from April 2011 to November 2012. The cases in 2011 were set as controls （ n = 68） , and the cases in 2012 were enrolled in the study group. The control group received the routine nursing care, and the study group received functional exercises besides routine nursing care. The patients＇ compliance to functional exercises, postoperative complications and patients＇ satisfaction to nursing care were analyzed. Results The patients＇ compliance to functional exercises in the study group was higher than that in the control group （P 〈 0.05）. There were 60 cases with complete compliance, and 8 with incomplete compliance during hospitalization in the study group. While in the control group, there were 20 cases with complete compliance, 42 with incomplete compliance, and 6 with complete non-compliance during hospitalization. The lower extremity venous thrombosis occurred in 6 cases in the control group, while no case in the study group （ P 〈 0.05 ）. Conclusion Implementaion of periodic functional exercises can improve compliance to functional exercises in patients with lumbar intervertebral disk protrusion, reduce postoperative complications, and promote rehabilitation
