小儿脾切除76例临床分析

目的总结小儿脾切除术的近期疗效,以提高手术安全性,减少并发症,并指导小儿脾切除术的适应征。方法回顾性分析76例实行脾切除手术患儿的临床资料,包括切脾指征、术后并发症及治疗效果等。结果本组平均年龄8.3岁(2 d～14岁),以外伤性脾破裂为最多(33例,43.42%),其次为血液代谢性疾病(20例,26.32%)。均行全脾切除术,21例(27.63%)同时行门静脉断流术,25例同时行(32.89%)肝脏活检术。术后发生并发症11例次(14.47%),以感染(8例)为最多,无死亡病例。结论充分认识脾脏的免疫功能,正确掌握适应征,加强围手术期处理是提高小儿脾切除手术安全性的关键。

Clinical analysis of children splenectomy

Objective To summarize the children Splenectomy in the near furore effect to improve their safety,reduce complications,and to provide guidance of indications to children Splenectomy.Methods A retrospective analysis of 76 cases of children with Splenectomy clinical data,including indications of splenectomy,the postoperative complications and the therapeutic efficacy.Results The children splenectomy,for the most traumatic rupture (33 cases,43.42%),followed by blood metabolic diseases (20 cases,26.32%).At the same time,21 cases of patients with portal vein devascularization,25 cases with liver biopsy.11 cases occurred after (14.47%) complications,infection (8 cases),The number of non-death cases.Conclusions It is the key that fully understand the immune function of the spleen,correctly grasp the indications to enhance perioperative management is to improve the safety of children underwent splenectomy.
