30例喉外伤临床观察

目的探讨喉外伤的诊断与治疗。方法回顾性分析我科收住30例喉外伤的诊治情况。结果30例均存活。闭合性喉外伤14例中气管切开4例,喉气管修复术5例。其中1例气管损伤延误诊治2d出现窒息,紧急气管切开、气管修复抢救成功。开放性喉外伤16例中气管切开13例,喉软骨复位术7例。术后均顺利拔管,无喉狭窄。1例喉、颈段食道闭锁行全喉切除术,恢复吞咽功能。结论对喉外伤的及时诊断和处理有利于降低损伤所造成的后果,促进喉功能的恢复和防止喉狭窄的发生。对有喉狭窄者,应选择适当手术方式以免再次狭窄。

Clinical analysis of the laryngeal injury

Objective To review and evaluate the outcome of the treatment of external laryngeal trauma.Methods A retrospective study on 30 cases with external laryngeal trauma treated in our Hospital.Results All of them were survived.Among 14 patients with blunt laryngeal trauma,4 of them were treated by incising trachea,5 treated with laryngoplasty.One case of blunt trachea injury presented asphyxia after expectant treatment for two days and was performed by tracheotomy and the repairing trachea.Among 16 patients of penetrating laryngea trauma,13 were treated with the incision of Trachea,7 by laryngoplasty.One case of laryngeal stenosis and esophagus stenosis was treated with the total laryngectomy and restored the function of deglutition after operation.All of them were extubated without the complication.Conclusion Quick diagnosis and adequate therapy can reduce the injuries,increase the chances of complete recovery of laryngeal function and prevent the stenotic sequelae.If Laryngostenosis occur,modus operandi should be chosen appropriately in order to avoid restenosis.
