股骨缩短术在大龄先天性髋关节脱位治疗的应用与实验研究

以常规行股骨缩短（ＦＳ）作为辅助手段，治疗大龄先天性髋关节脱位（ＣＤＨ）２２例２４髋。按改良Ｔｒｅｖｏｒ综合积分评定标准，优良率为８７．５０％，临床检查无明显肢长失衡。通过动物实验证实，ＦＳ可明显降低髋周软组织张力（Ｐ〈０．０１）及复位后头臼间接触压（Ｐ〈０．０１），为ＦＳ在大龄ＣＤＨ治疗中的应用提供了理论基础。

CLINICAL AND EXPERIMENTAL RESEARCH OF FEMORAL SHORTENING IN TREATMENT OF CONGENITAL DISLOCATION OF HIP IN ADOLESCENTS

Femoral shortening as a fundamental step in its treatment is described in this article.22 patients with 24 hips were followed up for 8 months to 11 years and 8 months, averaged 4 years and 11 months. According to the modified rating system of trevor,87 5% of the whole series had a good or excellent result.No significant leg length inequality was found in clinical examination.The results proved that the muscle tension around hip and contact pressure between femoral head and accelabulum had decreased markedly after femoral shortening used as an aid precedure in combined open reduction.It is suggested that femoral shortening might be used as a fundamental precedure routinely in the treatment of congenital dislocation of hip in older children and adolescents.
