经皮冠状动脉成形术术前禁食与术后低血糖反应相关因素的探讨

目的　为探讨术前禁食方法与术后低血糖反应的相关因素。方法　采用随机抽样法将择期行经皮冠状动脉成形术 (PTCA)的 10 0例病人分成 2组 ,对照组 5 0例病人术前 4h禁食 ;观察组 5 0例病人术前 2h仍可进食流质 ,血糖资料为术前空腹血糖和介入治疗后微量血糖 ,Whipple三联征为低血糖初步诊断依据。结果　观察组比对照组病人的并发症明显减少 ,2组不同年龄段PTCA术后发生低血糖有明显差异。 2组无 1例因反流而导致误吸。结论　对于择期PTCA的病人 ,术前 2h仍可进流质饮食 ,对于不能进食或手术时间较长的可用静脉补液的方法来补充能量 ,以减少低血糖反应

Study of Effective Factors on PTCA Patient's Forbiden Eating Preoperative and Hypoglycemia Reaction Postoperative

Objective Study the related factors between preoperative fasting and hypoglycemic reaction. Method Divide elective 100 PTCA patients into two groups by random sampling. 50 patients in comparison group fast 4 hours before operation while 50 patients in observation group eat fluid food even 2 hours before the operation. Preoperative fasting blood-sugar level and trace blood sugar after intervention therapy are recorded as the data of blood sugar. Use trilogy of Whipple as the basis for preliminary diagnosis of hypoglycemia. Result Complications of patients in observation group are significantly less than those in comparison group. Significant difference can be found between the incidences of hypoglycemic reaction from different-aged patients in two groups. No case of Aspiration is resulted from reflux in two groups. Conclusion Elective 100 PTCA patients can have fluid food 2 hours before the operation. For those who are unable to eat food or will have long operation can be intravenously injected to complement energy and correspondingly reduce hypoglycemic reaction.
