介入诊断和治疗在消化道出血中的应用价值

目的：探讨介入诊断和治疗在消化道出血中的应用价值。方法：32例消化道出血病例进行了数字减影血管造影(DSA)，19例患者采用动脉药物灌注治疗或动脉栓塞治疗。结果：DSA检查阳性19例占60％。其中肿瘤7例，动脉瘤8例。弥慢性肠黏膜病变和球溃疡出血3例，Crohn’s病1例。13例DSA检查阴性占40％。对19例(包括DSA阴性7例)采取动脉栓塞或血管加压素灌注治疗，10例得到完全止血，9例止血不彻底。结论：选择性动脉造影对不明原因消化道出血的定位和定性诊断有重要的价值，动脉血管加压素灌注和栓塞治疗是安全、有效和微创的止血措施。

Value and clinical application of interventional diagnosis and therapy in digestive tract bleeding

Objective:To study the value and clinical application of interventional diagnosis and therapy in digestive tract bleeding.Methods:32 patients with diagestive tract bleeding accepted digital subtract angiography (DSA).According to find out courses and position of bleeding during DSA,19 cases of 32 accepted arterial embolization and/or perfusion of vasoconstrictor substance.Results:DSA showed positive result in 19 cases (96%),of which,neoplasm was seen in 7,aneurysm in 8,diffuse mucosal lesion and duodenal ulcer in 3,and Chrohn's disease in 1.DSA showed negative result in 13 cases (40%).Arterial embolization or perfusion of vasoconstrictor was made in 19 patients (including 7 negative ones),of which the bleeding was completely stopped in 10,and partially controlled in 9.Conclusion:Selective angiogrphy is of great vlue in determining the location and nature of the digestive tract bleeding of unknown reasons.Arterial embolization and perfusion of vasocconstrictor is an effective method in the treatient of digestive tract bleeding.
