多维度护理在强直性脊柱炎合并严重颈胸段后凸畸形行截骨矫形内固定术中的应用效果

目的探讨多维度护理在强直性脊柱炎合并严重颈胸段后凸畸形截骨矫形内固定术中的应用效果。方法选取2017年9月至2018年9月入住我院的经截骨矫形内固定术治疗的强直性脊柱炎合并严重颈胸段后凸畸形患者52例为研究对象,根据护理方式将其分为对照组和研究组,各26例。对照组进行常规护理,研究组进行多维度护理。比较两组患者疼痛评分、并发症发生情况、生活质量和心理状态评分。结果干预前,两组患者疼痛评分比较,差异无统计学意义(P>0.05);干预后1、3、5个月,研究组疼痛评分均低于对照组,差异具有统计学意义(P<0.05)。对照组并发症总发生率高于研究组,差异具有统计学意义(P<0.05)。干预前,两组生活质量、SAS、SDS评分比较,差异无统计学意义(P>0.05);干预后,两组角色功能、认知功能、社会功能、躯体功能评分均升高,SAS、SDS评分均降低,且研究组角色、认知、社会、躯体功能评分均高于对照组,SAS、SDS评分低于对照组,差异具有统计学意义(P<0.05)。结论经截骨矫形内固定术治疗的强直性脊柱炎合并严重颈胸段后凸畸形患者的护理过程中应用多维度护理效果显著,值得在临床推广。

Application effect of multi-dimensional nursing in ankylosing spondylitis combined with severe cervical and thoracic kyphosis treated with osteotomy and orthopedic internal fixation

Objective To investigate the application effect of multi-dimensional nursing in ankylosing spondylitis combined with severe cervical and thoracic kyphosis treated with osteotomy and orthopedics internal fixation. Methods A total of 52 patients with ankylosing spondylitis combined with severe cervical and thoracic kyphosis treated with osteotomy and orthopedics internal fixation in our hospital from September 2017 to September 2018 were selected as the study objects, and they were divided into control group and study group by nursing mode, with 26 cases in each group. The control group who underwent routine nursing, and the study group who underwent multi-dimensional nursing. The pain scores, complications, quality of life and mental state scores of the two groups were compared. Results Before intervention,there was no significant difference in pain score between the two groups(P>0.05);1, 3, 5 months after intervention, the pain score of the study group was lower than that of the control group, the differences were statistically significant(P<0.05).The total incidence of complications in the control group was higher than that in the study group, the difference was statistically significant(P<0.05). Before intervention, there were no significant differences in quality of life, SAS and SDS scores between the two groups(P>0.05);after intervention, the scores of role, cognition, social and body function in the two groups increased, SAS and SDS scores decreased, and the scores of role, cognition, social and body function in the study group were higher than those in the control group, SAS and SDS scores were lower than those in the control group, the differences were statistically significant(P<0.05). Conclusion The application of multi-dimensional nursing in the nursing process of patients with ankylosing spondylitis and severe cervical and thoracic kyphosis treated with osteotomy and orthopedics internal fixation is significant and worthy of clinical promotion.
