Latent tuberculosis infection and preventive treatment attitude among elderly diabetic patients in Changning District of Shanghai

Objective To investigate the status of latent tuberculosis infection in elderly patients with diabetes mellitus in Changning District and their willingness of preventive treatment,and we provide basic data for the adjustment of tuberculosis screening strategy and mode in high-risk groups of tuberculosis in Changning District. Methods 60-79 years old patients with diabetes who participated in community physical examination from September to December in 2019 were selected. γ-interferon release test was used to detect tuberculosis infection, and the sampled population was surveyed on the basic information, health status, life behavior related factors, physical examination results, diabetes diagnosis and treatment and course of disease information, blood glucose control, tuberculosis core knowledge and LTBI knowledge awareness and preventive treatment attitude. SPSS 22.0 software was used to analyze the data. The rate comparison and multi-factor analysis were performed by χ² test and Logistic regression. P<0.05 was statistically significant difference. Results The latent infection rate of tuberculosis(LTBI) in elderly diabetic patients was 17.21%, patients with a history of hypertension had a lower risk of infection than people without a history of hypertension (OR=0.557; 95%CI: 0.328-0.947); the acceptance rate of preventive treatment in patients with LTBI was 60.81%. Conclusion The LTBI rate in elderly patients with diabetes mellitus in Changning District is lower than that of other domestic research reported. The patients with LTBI have a lower willingness to take preventive treatment. Knowing the relevant knowledge of tuberculosis and latent infection of tuberculosis plays a positive role in receiving preventive treatment.
