持续质量管理在门诊抽血室抽血患儿护理质量管理中的应用

摘    要:目的:探讨持续质量管理在门诊抽血室抽血患儿护理质量管理中的应用效果。方法:选择2018年12月1日~2019年12月1日门诊抽血室抽血患儿3000例作为研究对象。将2018年12月1日~2019年5月31日实施常规护理的1500例患儿作为对照组,将2019年6月1日~12月1日在常规护理基础上实施持续质量管理的1500例患儿作为观察组。比较两组采血等待时间和临时添加物品耗时、采血情况、纠纷发生情况、血液采集样本相关情况、采血配合度、采血不良反应及家属护理满意度。结果:观察组采血等待时间、临时添加物品耗时均短于对照组(P<0.01);观察组溶血率、凝血率、纠纷发生率均低于对照组(P<0.01),穿刺成功率高于对照组(P<0.01);观察组抽血失败率、血液样本不合格率、提前抽血标本、试管错误、登记错误、样本遗失、漏抽血标本发生率均低于对照组(P<0.01);观察组采血配合度高于对照组(P<0.01);观察组采血不良反应中皮下淤血、局部血肿、明显疼痛、出血发生率均低于对照组(P<0.01);观察组家属护理满意度调查问卷中穿刺技术、健康教育、服务态度、室内环境、排队等待评分均高于对照组(P<0.01)。结论:持续质量管理在门诊抽血室护理质量管理中的应用效果较好,其能有效缩短患儿等待及临时增加物品耗时,同时降低采血及血液样本不合格事件发生率,降低采血不良事件发生率,进一步提高患儿采血配合度及家属护理满意度。

Abstract:Objective:To explore the application effect of continuous quality management in nursing quality management of infant patients in outpatient blood drawing room.Methods:A total of 3000 children from the outpatient blood sampling room during December 1,2018-December 1,2019 were selected as research objects.Among them,1500 children with continuous quality management during December 1,2018-May 31,2019 were selected as the control group,and 1500 children with continuous quality management were selected as the observation group during June 1,2019-December 1,2019.Comparison was made between the two groups on the waiting time of blood collection and the time of temporary adding items in the two groups,the situation of blood collection,the occurrence of disputes,the qualification of blood samples,the matching degree of blood collection,and the adverse reactions of blood collection and nursing satisfaction.Results:The waiting time of blood collection and the time of temporary addition in the observation group were shorter than those in the control group(P<0.01).The hemolysis rate,coagulation rate and disputes in the observation group were lower than those in the control group(P<0.01),and the success rate of puncture in the observation group was higher than that in the control group(P<0.01).The failure rate of blood sampling,the unqualified rate of blood samples,the occurrence of early blood samples,test tube errors,registration errors,sample loss and missed blood samples in the observation group were lower than those in the control group(P<0.01).The matching degree of blood collection in the observation group was higher than that in the control group(P<0.01).The incidences of subcutaneous congestion,local hematoma,obvious pain and bleeding in the observation group were lower than those in the control group(P<0.01).The scores of puncture technique,health education,service attitude,indoor environment and queuing waiting in the nursing satisfaction questionnaire of the observation group were higher than those of the control group(P<0.01).Conclusion:The continuous quality management has good application effect in nursing quality management of outpatient blood sampling room,which can effectively shorten the waiting time of children and temporarily increase the time spent on items,reduce the incidence of blood collection and blood sample unqualified events,and reduce the incidence of blood collection adverse events,thus further improve children′s blood collection matching degree and nursing satisfaction.
