郭程湘采用双凤锤手法推拿治疗急性腰扭伤经验

总结了全国名老中医药专家传承工作室指导老师郭程湘医生运用双凤锤手法推拿治疗急性腰扭伤的经验。郭程湘医生将峨眉武术中的双凤锤手法引入至其畅气通络疗法推拿治疗中,用以治疗急性腰扭伤患者。治疗时,患者采用俯卧位,治疗部位集中在背部、腰部的督脉、膀胱经循行部位,即从督脉的大椎穴到腰俞穴、膀胱经的大杼穴到下髎穴这一循行路线。治疗时先采用抚法、掌背?揉、肘?法让患者充分放松,然后依据局部取穴和远端取穴相结合的原则,对腰阳关、命门、环跳、委中、承筋、承山等穴采用点穴治疗,再以双凤锤手法按压脊柱两侧的关节突关节,最后以腰部擦法、拍法结束治疗。与传统的腰椎斜扳法相比,该手法有一定特色且疗效确切,为临床推拿手法治疗急性腰扭伤尤其是因腰椎的滑膜嵌顿引起的急性腰扭伤提供了参考。

Experience of GUO Cheng-Xiang in the Treatment of Acute Lumbar Sprain with Shuangfengchui Manipulative Techniques of Massage

The experience of practitioner GUO Cheng-Xiang,one of the famous national Chinese medicine physicians for academic inheritance studio,in treating acute lumbar sprain with Shuangfengchui manipulative techniques of massage was summarized.Practitioner GUO Cheng-Xiang introduced the Shuangfengchui manipulative techniques in Emei Wushu to his massage therapy of regulating qi and unblocking collaterals,and applied the manipulative techniques for the treatment of patients with acute lumbar sprain.During the treatment,the patient was in prone position,and the manipulations were performed on the region along the meridians of governor vessel and bladder meridian on the back and waist,a route from Dazhui(GV14)to Yaoshu(GV5)of the governor vessel,and from Dazhu(BL11)to Xialiao(BL34)of the bladder meridian.At the beginning of the manipulative techniques,the patients were fully relaxed by using stroking manipulation and by using the kneading manipulation with the back of the palm and elbows.Secondly,the patients were treated with acupoint-pressing therapy on Yaoyangguan(GV3),Mingmen(GV4),Huantiao(GB30),Weizhong(BL40),Chengjin(BL56)and Chengshan(BL57)following the principles of selection of local acupoints combined with selection of distant acupoints,and then were given the Shuangfengchui manipulative techniques,pressing the facet joints beside the spinal column with the dorsal part of the middle knuckles of the second and the third fingers.Finally,the patients were given the rubbing and patting manipulations to complete the treatment.In comparison with the traditional lumbar oblique pulling manipulation,Shuangfengchui manipulative techniques of massage has its specific features and exert certain effect,which can provide evidence for the clinical massage treatment of acute lumbar sprain in particular for those accompanied by incarceration of the lumbar synovium.
