烧伤病人血清中补体旁路溶血活性变化

摘    要:本文用未致敏家兔红细胞测定Mg-EGTA 处理的人血清的补体旁路溶血活性,并用这一技术检测烧伤病人血清中补体旁路活性变化。总计观察40例住院烧伤病人,以48例正常人血清标本作为对照。结果表明(1)31例急性期烧伤病人血清的旁路活性低于正常(p<0.05),其中以24例脓毒症降低更为明显(p<0.001)。与此相反,非脓毒症(n=7)和恢复期血清(n=9)的旁路活性差异无显著性(p>0.05)。脓毒症与非脓毒症相比也发现差异有显著性(p<0.001)。(2)烧伤后最初二周,补体旁路活性低于正常(p<0.05),尤以前三天最明显(p<0.01)。(3)中、重度烧伤病人的旁路活性与正常人差异无显著性(p>0.05),而特重度烧伤病人旁路活性明显降低(p<0.05)。以上结果提示烧伤病人血清补体旁路活性变化与感染和烧伤严重程度相关。

Abstract:Hemolytic activity of the alternative complement pathway(ACP)of human serum treated with Mg-EGTA was measured using unsensi-tized rabbit erythrocytes.The sera of 40 burn patients were analy-zed using this technique,48 normal human serum samples taken ascontrol.The general results were as follows:1.The ACP of the sera of 31 acute-phase burn patients,wassignificantly diminished in comparison with the control.In 24 pyemicstate burn patients,the decrease was even more profound(P<0.001).The ACP activities of sera from both 7 non-pyemic and 9 convales-cent phase burn patients were elevated,it is not statistically significant(P>0.05).A significant difference was noticed between the pyemicand non-pyemic patients(P<0.001) .2.In all of the burn patients studied,the ACP activity was significantly depressed during the first 2 postinjury weeks(P<0.05).The depression was most obvious during the first 3 days(P<0.01).3.The ACP activity was below normal in patients with modera-tely extensive burns,but the only statistically significant decreasewas seen in the most severely burned cases(P<0.05).The observations of the changes in ACP activity showed a correlationwith the presence of infection and the severity of the burn injury.
