综合措施预防早产儿发生坏死性小肠结肠炎的临床研究

目的探讨预防早产儿发生坏死性小肠结肠炎的有效方法。方法将我院NICU2003～2005年度收治的所有早产儿进行综合管理,包括:谨慎喂养;免疫支持;积极抗感染;及时改善肠道微循环、微生态;循环、呼吸、血糖、内环境支持等;尽早给予静脉营养支持以保证热卡供给;使用中心静脉置管输液;严格控制静脉入液量。观察以上综合措施实施后,早产儿坏死性小肠结肠炎的发生情况。结果308例早产儿中,共发生坏死性小肠结肠炎3例,发生率为0.97%,低于文献报道的在所有NICU病婴中总的发生率1%～7.7%水平。结论早产儿综合管理的综合预防措施对减少早产儿坏死性小肠结肠炎的发生有积极的意义。

Clinical Study of the Comprehensive Measures for Preventing Premature Infant Necrotizing Enterocolitis

Objective Studies the comprehensive measures for preventing premature infant necrotizing enterocolitis.Method From 2003 to 2005,all premature infants admitted our NICU were treated with comprehensive measures.Including careful feeding,immunoenhancement,active anti-infection,prompt improvement of the microcir-culation and microecosystem of the gut,support of circulation,respiration,blood glucose and the inside environment,early supply of parenteral nutrition,use central venous delivery,strictly control the intravenous deliver rate.The incidence of necrotizing enterocolitis was observed among these infants treated with the comprehensive measures.Rusult In 308 premature infants,only 3 had necrotizing enterocolitis,with an incidence rate of 0.97%,which is lower than the rate occurred in NICU reported in literature(1% to 7.7%).Conclusion The comprehensive measures can significantly reduce the incidence of premature infant necrotizing enterocolitis.
