2型糖尿病胰岛素强化治疗对血脂变化的临床研究

目的探讨2型糖尿病胰岛素强化治疗对血脂的影响。方法选取120例2型糖尿病患者,随机分为实验组和对照组,分别给予持续皮下胰岛素输注(CSII)和多次皮下胰岛素注射(MSII)治疗,比较两组患者治疗结果和治疗前后血脂变化情况。结果实验组胰岛素用量、空腹血糖、餐后2h血糖、血糖达标天数及低血糖发生率均显著低于对照组,差异有统计学意义(P<0.001);两组治疗后TC、TG水平均明显下降,差异有统计学意义(P<0.05);HDL-C水平均明显上升,实验组改善更明显,差异均有统计学意义(P<0.05)。结论胰岛素泵给药强化治疗在有效控制血糖的同时,明显改善患者的血脂,重视糖尿病强化治疗的作用有重要意义。

The intensive insulin therapy of Type 2 diabetes on serum lipid changes

Objective To investigate the effect of intensive insulin treatment of type 2diabetes with insulin on blood lipid changes. Methods A total of type 2diabetic patients were randomly divided into an experimental group and a control group,patients in the experimental group were treated with continuous subcutaneous insulin infusion（CSII）and patients in the control group received multi- subcutaneous insulin injection（MSII）treatment.The clinical results and pre- and post-treatment blood lipid changes were compared between the experimental group and the control group. Results Patients in the experimental group had lower dosage of insulin,lower fasting blood glucose,lower 2hpostprandial blood glucose,less time to reach target and lower incidence of hypoglycemia than the control group（P〈 0.001）,both TC and TG levels in the two groups decreased significantly after treatment,compared to pre-treatment,there was statistically significant difference（P〈0.05）.HDL-C levels were significantly increased,the experimental group improved more obviously,the difference was statistically significant（P〈0.05）. Conclusion Intensive insulin pump treatment can effectively control of blood glucose as well as improving blood lipid,it has important significance of paying attention to the efficacy of intensive treatment of diabetes.
