低剂量64层螺旋CT在颌面部外伤中的临床应用

摘    要:目的探讨并分析低剂量64层螺旋CT诊断技术在颌面部外伤骨折中的应用和效果,为临床相关病例的诊治提供依据和相关参考。方法随机选取于2013年3月2013年12月间我院收治颌面部外伤患者的临床资料进行回顾性分析,共计61例。所有患者入院诊断时均采用低剂量64层螺旋CT扫描以及图像重建处理技术,完成CT扫描以及图像重建后,与X线拍摄平片进行对比,验证、确定伤情。结果低剂量64层螺旋CT在颌面部外伤61例诊断中,发现眼眶骨折13例、颧骨19例、上颌骨24例、下颌骨35例、鼻骨9例和筛骨3例。CT与X线平片诊断基本相符,但X线平片未能检出上颌骨与鼻骨的复合骨折。两者诊断眼眶、上颌骨、下颌骨、鼻骨复合骨折差异有统计学意义(P均<0.05)。结论低剂量64层螺旋CT扫描技术在诊断患者骨折伤势方面具有明显的优势和良好的效果,为病情的诊断和处理提供有理的依据,同时也为临床医师在后续治疗、设计手术方案、患者恢复方案等过程提供重要信息及参考,值得临床借鉴和广泛推广。

Abstract:Objective To investigate and analyze the diagnostic value of low dose 64-slice spiral CT in maxillofacial trauma for providing evidences and reference in clinical treatment. Methods Clinical data of 61 patients with maxillofacial traumatic fractures cared by our hospital March 2013 to December 2013 were retrospectively analyzed.All patients received low dose 64-slice spiral CT scanning with image reconstruction processing technology when admission to hospital,and after completing CT scanning and image reconstruction,the scanning presentation were compared with X-ray plain films for identifying disease condition. ResultsThe low dose 64-slice spiral CT scanning found that orbital fracture occurred in 13 of 61patients with maxillofacial trauma,fracture of zygomatic bone in 19 cases,fracture of maxilla in 24 cases,fracture of mandible in 35 cases,nasal bone fracture in 9cases and ethmoid fracture in 3cases.CT diagnosis was appropriately in line with X-ray plain films,but X-ray could not detect complex fractures of maxilla and nasal bone.Compared the accuracy rate of diagnosing complex fractures of orbit,maxilla,mandible and nasal bone between the low dose 64-slice spiral CT and X-ray plain films,there were statistically significant differences(all P 0.05). Conclusion Low dose 64-slice spiral CT has obvious advantages and good results in diagnosis of patients with fracture injury,provides reasonable evidences for disease diagnosis and treatment,as well as for clinicians in the subsequent treatment,design procedure,and provides important information and reference in patients recovery program process,which is worthy of reference for clinical and widely available.
