原发性干燥综合征肺部病变患者47例临床表现和实验室指标分析

目的探讨原发性干燥综合征（pSS）肺部病变患者的临床表现和实验室检查的特点。方法对近年来浙江大学医学院附属第二医院收台的pSS（129例）伴肺部病变者47例的临床表现和实验室特点与无肺部病变者（82例）进行比较分析。结果肺部病变患者的年龄大于无肺部病变者．病程长于无肺部病变者（P〈0．05或0．01）；肺部病变患者呼吸道症状中咳嗽、咳痰和胸闷气促的发生率均显著高于无肺部病变者（均P〈0．01）；肺部病变者的抗SSA抗体阳性率、免疫球蛋白G水平、类风湿因子滴度、低补体血症发生率均高于无肺部病变者（均P〈0．05或0．01）。结论pSS肺部病变发生率较高。且表现形式呈多样化；年龄较大、病程较长、抗SSA抗体阳性、高球蛋白血症、高滴度的类风湿因子、低补体血症等均提示其并发肺部病变的可能。

Correlation of clinical manifestation with laboratory examination in patients with lung involvement of primary sjogren's syndrome

Objective To investigate the correlation of clinical manifestation with laboratory examination in patients with lung involvement of primary Sjogren＇s syndrome （pSS）Methods One hundred and twenty nine patients with PSS were admitted in the hospital during January 2006 and January2010, the diagnosis was made according to the 2002 international classification for pSS. Lung involvement was detected by chest imaging （HRCT） in 47 patients （36.4%）, chronic obstructive pulmonary disease, asthma, congenital heart disease, rheumatic heart disease and other conditions were excluded. The clinical manifestation and results of laboratory examination were compared between patients with and without lung involvement. Results Age, duration of the disease, Anti-SSA antibody-positive, immunoglobulin G, RF titer, low complement in patients with lung involvement of primary Sjogren＇s syndrome were significantly higher than those in patients with no lung involvement （P 〈 0.005） . Conclusion Lung involvement in pSS is common and clinical manifestations are various. The advanced age, long duration of the disease, anti-SSA antibody-positive, rising of RF titer, immunoglobulin G and the low complement in PSS patients may suggest the lung involvement.
