可调整缝线在复合式小梁切除术中的应用

目的观察巩膜调整缝线在应用丝裂霉素c的小梁切除术中对减少术后浅前房和控制眼压方面的作用。方法将43例(47眼)各种类型青光眼患者随机分组,实验组22例(24眼)行常规小梁切除术,术中应用丝裂霉素c 巩膜可调整缝线;对照组21例(23眼)行常规小梁切除术,术中应用丝裂霉素c。对其术后浅前房、滤过泡及眼压进行观察。结果实验组术后浅前房发生率较对照组少,但差异无显著性(精确概率法,P=0.072),有待进一步观察;眼压控制及功能性滤过泡比较,差异均无显著性。结论巩膜可调整缝线在应用了丝裂霉素c的复合式小梁切除术中应用可调节房水滤过量,减少术后浅前房和低眼压的发生,提高手术成功率。

Application of the adjustable sutured scleral flap in the composite trabeculectomy

Objective To evaluate the effect of adjustable sutured scleral flap on reducing shallow anterior chamber and controlling intraocular pressure in the trabeculectomy supplemented with MMC.Methods Forty-three cases(47 eyes) of various forms of glaucoma were randomly divided into two groups.In Group A,24 eyes of 22 consecutive patients underwent regular trabeculectomy,supplemented with MMC and the adjustable sutured scleral flap.In Group B,23 eyes of 21 consecutive patients underwent regular trabeculectomy supplemented with MMC only.The shallow anterior chamber,filtering blebs and intraocular pressure of the two groups were evaluated and compared.Results The occurrence rate of the shallow anterior chamber in Group A was lower than that of Group B,but there was not statistically significant difference between the two groups(Fisher's Exact Test P=0.072). The functional filtering blebs and intraocular pressure between the two groups were also not significantly different.Conclusion Applying adjustable sutured scleral flap to the trabeculectomy supplemented with MMC may adjust the amount of aqua oculi out-flowing,reduce the occurrence of the shallow anterior chamber and low intraocular pressure,and thus improve the succeed rate of trabeculectomy.
