外伤性颅内血肿的保守治疗(附117例报告)

本文报告了117例外伤性颅内血肿的保守治疗和随访结果,其中男89例,女28例,年龄2月～76岁,平均40岁。所有病例均经CT扫描和追踪观察。结果表明,血肿量(V)<20ml89例,保守治愈;20ml40ml 12例,中转手术4例,其中2例死于术后并发症,死亡率为1.71%.对115例进行了随访,102例恢复了劳动或学习,余13例均能正常生活。作者结合文献讨论了外伤性颅内血肿保守治疗的适应症,并提出了5条保守治疗的指征。

CONSERVATIVE TREATMENT OF TRAUMATIC INTRACRANIAL HEMATOMA:A REPORT OF 117 CASES

The results of conservative treatment of 117 cases of traumatic intracranialhematoma in our hospital from October 1984 to December 1989 are reported. Thepatients included 89 males and 28 females, aged from 2 months to 76 years, avre-aging 40 years, CT carried out in all the cases showed small hematoma (V<20ml)in 89 patients with whom conservative treatment was successful; mediumsizedhematoma (20ml40ml) in 12 patients, of whom 4underwent operation and 2 of them died from complications after operation (morta-lity rate, 1.71%) The average time for hematoma absorption was 36 days. Follow-up observation of 115 cases found 102 patienhts had fully recovered andreturned to work or school life. The other 13 had also returned to normal condition. We consider that selective conservative treatment of the patients in our series wassatisfactory. Indications to which conservative treatment is applicable are discussedand summarized in the light of litrature at home and abroad.
