MRI在肩关节外伤中的应用价值

目的:探讨MRI在肩关节外伤中的应用价值。材料和方法:对34例有明确肩关节外伤的病例,用1.5T MR仪行肩关节检查,分析MRI在肩袖撕裂、盂唇损伤的表现特点,并与手术或关节镜结果对比。结果:34例中25例经手术或关节镜证实,其中单纯冈上肌肌腱部分撕裂17例,完全撕裂2例,冈上肌和二头肌肌腱同时完全撕裂1例,同时部分撕裂5例,肩袖撕裂伴盂唇撕裂2例,术前诊断正确率分别为92%(肩袖撕裂)和80%(盂唇撕裂)。结论:MR多平面成像及良好的软组织分辨率,能详细显示肩关节周围的软组织结构,对肩关节外伤中的肩袖撕裂、盂唇损伤的诊断,有较高的应用价值。

The value of MRI in the diagnosis of shoulder trauma

Objective: To evaluate the value of MRI in the diagnosis of shoulder trauma. Materials and Methods: Thirty-four cases with a history of shoulder trauma underwent MR imaging. The results of MRI were studied retrospectively and compared with the findings of surgery and arthroscopy. Results: Twenty-five of 34 cases had been confirmed by surgery or arthroscopy. Seventeen cases were pure partial tears of supraspinatus and two were complete tears, one was complete tear of supraspinatus and biceps at the same time and five were partial tears, two were rotator cuff tears and glenoid labrum tear at the same time. The accuracy rate preoperatively was 92%(rotator cuff tears) and 80%(glenoid labrum ruptures). Conclusion: MRI is a good modality in the diagnosis of rotator cuff tears and glenoid labrum ruptures of shoulder trauma.
