UAP患者应用低分子肝素钙的临床意义及护理

目的探索不稳定型心绞痛（UAP）患者应用低分子肝素钙的临床疗效和护理方法。方法70例不稳定型心绞痛患者随机分为A、B两组。B组常规应用硝酸脂类、β-受体阻滞剂、ACEI制剂、调脂药物、阿司匹林等治疗,A组在常规治疗的同时加用低分子肝素钙皮下注射,疗程7~10天,观察两组心绞痛发作及心电图心肌缺血改善情况、急性心肌梗死（AM I）发生率、出凝血指标监测、出血并发症情况等。结果A组与B组比较,心绞痛缓解有效率分别为74.3%、91.4%（P〈0.05）,心电图恢复有效率为68.6%、88.6%（P〈0.05）,AM I发生率为14.3%、2.9%,两组差异均有显著性。两组出凝血时间与血小板检测无明显差异（P〉0.05）。结论低分子肝素治疗UAP并结合适当护理,疗效肯定、安全,可显著降低AM I发生率。

The clinical significance and nursing of low molecular heparin calcium in patients with unstable angina pectoris

Objective： To observe the clinical significance and nursing of low molecular weight heparin calcium in the treatment of unstable angina pectoris.Methods： 70 cases of unstable angina pectoris were randomly divided into two groups,group A（n=35） and group B（n=35）,The group B was treated with routine therapy,including Nitric、β-blocker、ACEI、 statin and aspirin.while the group A was treated with low molecular heparin calcium on the bases of routine therapy.Results： The effective rate of angina pectoris occured in group A and group B was 74.3 % vs 91.4%,and the ECG improvement rate was 68.6 % vs 88.6%,the AMI occurrence rate was 14.3% vs 2.9%,there were significant difference between two groups（P〈0.05）.The test of PT and platelet showed little difference between two groups（P〉0.05）.Conclusion： The application of low molecular heparin calcium combining appropriate nursing may treat UAP effectively with safety on PT and platelet,alleviate the pain symptom,decrease the AMI occurrence rate significantly during acute phrase of UAP.
