血清心肌钙蛋白T在烧伤患者中的诊断价值

摘    要:目的 评估血清心肌钙蛋白T对烧伤后心肌损伤的诊断价值。方法 将72例单纯热力烧伤患者用伤情分类方法分为轻中度蛆(31例)、重度组(25例)、特重度组(16例),将15例正常体检人员为对熙组。患者入院后1、7、14天采集周围静脉血,同时行心屯图和超声心动图检查。结果 (1)重度及特重度组患者在伤后各时相点的cTnT值均高于轻中度烧伤组和正常对照组,最高可这正常组的60倍,而且伤情越重,cTnT升高幅度也越大。(2)CK-MB与cTnT变化规律基本一致,但升高幅度较低,只有正常对照组的3~6倍;伤后14天。轻中度与重度组相盖无显著性意义。结论 本组资料显示,重度和特重度组烧伤患者中有较明显的cTnT异常升高,最高达正常对照组的60倍左右,而且幅度与伤情轻重相一致,伤情越重,升高幅度也越大。升高的幅度比CK-MB大,持续时间长。cTnT能更好地反映严重烧伤患者的心肌损伤状态,具有更强的特异性。

Diagnosing Value of Serum Myocardial Calcium Protein T in Patients with Burn

Abstract:Objective To evaluate the diagnosing value of serum myocardial calcium protein T for heart muscle damage after burn.Methods 72 cases with heat burning were divided into 3 groups according to the hurn degree:31 cases in medium group,25 cases in severe group and 16 cases in the most severe group,in the control group,there were 15 healthy cases.Peripheral venous blood was collected 1 day,7 days and 14 days after admission and ECG and UCG were made.Results cTnT of the patients in severe and the most severe groups were higher,or even 60 times,than those in medium group and the control group,the more severe the burn was,the bigger the increasing amplitude of cTnT was;CK-MB had the almost same change with cTnT but is increasing amplitude was smaller,3 to 6 times than that in the control group.14 days after burning,patients in medium group showed out no significant differences from those in the severe group.Conclusions cTnT of patients in severe and the most severe group rose greatly,the increasing amplitude was bigger than that o CK-MB.cTnT may better display the heart muscle damage of the patients with severe burn.
