胃术后晚期出血的原因及治疗

本文分析了1967年3月～1984年12月收治的35例胃良性疾病术后晚期出血的原因。其中以复发性溃疡伴出血最多见(62.9%),其次为返流性胃炎出血(20%)。平均随访观察9.5年(2.5～19年),无1例死亡。17例复发性溃疡出血保守治疗中15例发生再出血,而17例手术止血者仅4例再次发生术后晚期出血。提示复发性溃疡出血者应手术治疗;返流性胃炎出血保守治疗效果较好,当保守治疗失败或反复出血者应手术治疗。

Causes and Treatment in late Stage of Postoperative Gastric Hemorrhage

35 cases of hemorrhagic causes in the late stage of postoperative gastric benign dise- ases admitted from Match 1967 to December 1984 are analyzed.The first cause is recurrent ulcer(62.9%),the second is regurgitant gastritis(20.0%).The follow-up peried is average 9.5 years(arange 2.5～19 years)without any case died in this series.Among 17 cases of the hemorrhage due to recurrent ulcer with nonoperative therapy,bleeding occured again in 1.5 cases;but those with operative therapy,bleeding did only in 4 cases.It suggests that the hemorrhage due to recurrent ulcer shotld be treated operatively,and that the hemorrage due to regurgitant gastritis had better treated in conservative therapy.If failed or gastrorrhagia recurred,it is ought to decide for operation.
