食管癌及癌前病变中p53、CD44v6蛋白的表达及其意义

摘    要:目的 定位分析p5 3、CD44v6蛋白在食管正常黏膜、单纯增生、不典型增生、原位癌及浸润癌中的表达及与食管癌病理特征的关系。方法 采用免疫组化方法检测p5 3蛋白、CD44v6蛋白在 45例原发性食管鳞癌大体标本中的不同病变区域的表达。结果 p5 3蛋白在食管癌自然病史中呈现渐进性高表达 ,而CD44v6蛋白则在其中呈现渐进性低表达。p5 3蛋白表达在单纯增生和不典型增生中有显著性差异 (P <0 0 1) ,CD44v6蛋白则在原位癌与浸润癌中存在显著性差异 (P <0 0 1)。p5 3蛋白在食管癌自然病史中的渐进性高表达与CD44v6蛋白在其中的渐进性低表达有相关性 (P <0 0 1) 结论 p5 3蛋白表达和CD44v6蛋白表达的改变与食管癌自然病史密切相关 ,可以作为判断食管癌生物学行为的指标

Abstract:Objective To analyse orientationally the expressions of mutant p53 and CD44v6 proteins in the esophageal normal mucosa, hyperplasia, dysplasia, carcinoma in situ and invasive carcinoma Methods Immunohistochemistry was demonstrated to show these tumor markers' expression in different epithelial lesions Results Stepwise over_expression of mutant p53 and reduction of CD44v6 protein's expression were observed in the multistep process of esophageal carcinogenesis Significant difference in expression of p53 (P <0 01) was observed between hyperplasia and dysplasia; while expression of CD44v6 ( P <0 01) in mucosal invasive carcinoma was significantly different from that in carcinoma in situ Moreover, stepwise over_expression of mutant p53 showed significant correlation with reduction of CD44v6 protein's expression Conclusion The expression of mutant p53 protein and CD44v6 is correlated with esophageal carcinoma progression and, therefore, may be important markers for delineating subsets of tumors with more aggressive biology
