脑积水病人侧脑室-横窦脑脊液分流术18例报道

摘    要:目的观察侧脑室-横窦CSF分流治疗脑积水的疗效和并发症。方法对18例脑积水病人,用美国Heyer-Schulte中压管行优势半球侧侧脑室-横窦CSF分流术。术后定期复查,并随访24个月。结果16例维持良好(89.4%),1例皮下积液继发感染(5.5%),1例脑室端分流管阻塞(5.5%)。颅内压基本维持在正常水平(60~140mmH2O)。脑CT显示8例脑室大小恢复正常;4例小于正常;5例稍大于正常,但较手术前明显缩小;1例增大(正常压力性脑积水)。未见横窦血栓形成,无死亡。结论侧脑室-横窦CSF分流术可作为临床治疗脑积水的方法。其远期疗效和并发症有待于进一步观察。

Abstract:[Objective] To observe the effect and complications of lateral ventricle-transverse sinus shunt in hydrocephalus. [Methods] Lateral ventricle-transverse sinus shunts were inserted in 18 hydrocephalic patients by medium-pressure catheters (Heyer-Schulte, USA), and the patients were observed in 24 months. [Results] Keep good results in 16 cases (89.4%), one case (5.5%) suffered effusion and inflammation under the head skin, and the catheter was taken out. In another case (5.5%), the drainage catheter was blocked. There was no thrombosis in transverse sinus, and no death. The ICP was general normal (60~140 mmH2O) in 16 of 18 cases. [Conclusion]lateral ventricle-transverse sinus shunt is proper method in the treatment of hydrocephalus. The effect and the complications should be observed in more cases and in longer term.
