儿童紫癜性肾炎肾内凝血和纤溶活性的检测及临床意义

目的　观察儿童紫癜性肾炎患者 (HSPN)尿中纤维蛋白肽A(FPA)和D 二聚体 (D dimer)的变化 ,探讨肾内凝血和纤溶活性与紫癜性肾炎发生发展的关系。方法　 5 9例儿童紫癜性肾炎患者 ,根据血肌酐值 (sCr)分为正常 (N)、低 (L)、中 (M)和高 (H) 4组 ,分别检测尿FPA和D dimer的含量。结果　四组患者尿中FPA和D dimer含量皆显著高于对照组 (P <0 .0 1) ,且两者与血肌酐呈显著正相关 (r值分别为 0 89和 0 85 ,P <0 .0 1)。结论　紫癜性肾炎患者肾内存在凝血和纤溶的激活 ,肾内凝血和纤溶活性的亢进促进了紫癜性肾炎的发展。

Detection of Activities of Renal Coagulation and Fibrolysis in Children with Henoch-Schonlein Purpura Nephritis and Its Clinical Significance

Objective To investigate the variation of urinary fibrinopeptide A (FPA) and D-Dimer in children with Henoch-Schonlein purpura nephritis(HSPN) patients and their relationship with HSPN incidence and development.Methods 59 children HSPN patients were divided into 4 groups according to serum creatinine (sCr) level as normal(N),low(L),middle(M) and high(H) groups.The level of urinary FPA and D-Dimer were examined.Results The level of urinary FPA,D-Dimer in 4 groups were significantly higher than that of normal control group (P<0.01),and two positive correlations were found between the level of the two and the serum creatine (r=0.89,0.85,P<0.01).Conclusion Renal activities of coagulation and fibrolysis have been displayed in children HSPN patients,and their activities contribute to the progress of HSPN.
