| 摘 要： | 目的评价超声引导经皮肾镜气压弹道联合超声碎石术治疗肾及输尿管上段结石的方法及疗效。方法回顾性分析2006-08～2007-11应用超声引导经皮肾镜气压弹道联合超声碎石术治疗97例肾及输尿管上段结石患者的临床资料,对其方法、临床疗效及手术并发症等资料进行评价。其中单发肾盂、肾盏结石27例,多发肾结石26例,肾鹿角形结石14例,其中双侧结石9例;单纯输尿管上段18例21侧,肾结石合并输尿管上段结石12例17侧。结石长径1.2￣6.0cm,采用B超定位建立经皮肾镜操作通道,EMS第三代气压弹道超声碎石机用于粉碎并清除结石。结果97例患者均一期成功建立皮肤肾脏通路,93例行一期碎石术,4例行二期碎石术。手术时间50￣180min,平均90min。术中出血量100￣400ml,平均150ml。结石清除率为76.2%(74/97)。无肾切除、腹腔脏器损伤、血气胸等严重并发症。结论超声引导经皮肾镜气压弹道联合超声碎石术治疗肾及输尿管上段结石微创、高效、安全、并发症少。
