双支撑钢板内固定治疗C型胫骨平台骨折的临床应用

目的　观察双支撑钢板治疗C型胫骨平台骨折的临床疗效。方法　将36例C型胫骨平台骨折患肢根据 AO分类法分类切开整复,胫骨内外侧同时使用“T”形或“L”形支撑钢板,松质骨螺钉内固定。关节面塌陷严重者行自体骨植骨支撑,术后早期应用CPM机活动膝关节。结果　根据Rasmussen的膝关节功能评价标准,优16例,良13例,中4例,差3例,优良率达80.6%。结论　胫骨双支撑钢板设计良好,较易完成复杂髁部骨折的内固定,骨折复位满意,关节面平整,内固定坚强,可早期功能锻炼,疗效满意,是理想的内固定方法。

Clinical application of internal fixation by double support plate in the treatment of the tibial plateau fracture of Type C

Objectives To discuss the clinic application of treating tibial plateau fracture of Type C with double support plate.Methods According to AO classification. All patient underwent open reduction and internal fixation. Both side of tibial plateau were fitted by double T or L type support plate and cancellous screw. Self bone grafting were used in serious articulus surface subside cases,and CPM was used early to keep knee joint active. The curative effects were observed. Results According to Rasmussen's classification,16 cases were excellent, 13 cases were better, 4 cases were poor,3 Cases were bad among these 36 cases. Excellent rate reached 80.6%.Conclusion The double supporting plate is well-designed, and easy to accomplish stable internal fixation. It can make articulus surface well-anatomized, and alignment. It can keep the knee joint the enable in early exercise. So it is a good choice of internal fixation.
