米索联合吸引套管用于剖宫产术后无痛人流120例的观察

目的探讨米索前列醇联合一次性宫腔吸引管用于剖宫产术后患者无痛人流的临床效果及并发症发生情况。方法选择门诊自愿要求实施无痛人流术的剖宫产术后患者360例,将其随机均分为3组。观察组120例患者术前2h口服米索前列醇600μg,使用一次性宫腔吸引套管进行无痛人流术;同等条件选择120例剖宫产术后患者作为对照组1,对照组1术前不予米索前列醇,采用常规金属吸头施行手术;另随机选择120例剖宫产术后患者作为对照组2,对照组2术前予以米索前列醇600μg口服,手术器械同对照组1。观察3组患者宫颈口扩张情况、镇痛效果及手术并发症情况。结果观察组、对照组2宫颈扩张程度、镇痛效果等指标结果均明显优于对照组1（P〈0.01）。在手术并发症方面,观察组、对照组2宫颈（宫腔）粘连、子宫穿孔、月经失调的发生明显少于对照组1（P〈0.01）,漏吸的发生3组无明显差异（P〉0.05）。观察组子宫穿孔发生明显少于对照组2（P〈0.05）;两组宫腔（宫颈）粘连、月经改变发生情况无明显差异（P〉0.05）。结论无痛人工流产术前2h口服米索前列醇600μg、术中联合采用一次性宫腔吸引管能有效扩张宫颈,增强麻醉效果,降低手术并发症,是一种比较理想的终止剖宫产术后患者早孕的方法。

Observation of Misoprostol Joint Attractive Casing in 120 Cases of Painless Cesarean Section

Objective To explore the clinical efficacy and complications of misoprostol combined to a one-time intrauterine attract pipe in cesarean section patients with painless abortion.Methods The 360 cases of cesarean implement a painless outpatient volunteers were selected and divided into 3 groups randomly.Observed group of 120 patients who were taken misoprostol 600μg orally 2h before operation,received the painless abortions with one-time intrauterine attract pipe;select 120 cases of cesarean section at the same conditions as the control group 1,control group 1 no misoprostol preoperativly,using conventional metal nozzle for operation;another 120 cases of cesarean section as the control group 2,patients in control group 2 were taken misoprostol 600μg orally,the surgical instruments is the same with the control group 1.The dilation of cervical,analgesia and surgical complications of 3 groups were observed.Results The cervical dilatation,analgesia and other indicators of the results of the observation group and control group 2 were significantly were better than control group 1（P0.01）.Surgical complications in the observation group and control group 2 such as cervical（uterine）adhesions,uterine perforation and the incidence of menstrual disorders were significantly less than the control group 1（P0.01）,the occurrence of suction drain in the 3 groups showed no significant（P 0.05）.The uterine perforation in observation group was less than the control group 2（P0.05）;There were no significant differences between the two group uterine（cervical）adhesions and menstrual changes in incidence（P0.05）.Conclusion To take 600μg misoprostol orally 2h before painless abortion operation,combined with one-time intrauterine attract pipe can dilate cervix efficiently and strengthen anesthesia,reduce the complications,which is an ideal method of termination the early pregnancy of patients after cesarean.
