断指再植适应证的定量诊断

目的 建立断指再植适应证的定量诊断方法，方法 取断指再植失败与成功者各40指，根据患者年龄，致伤类型，伤后就诊时间，断指保存情况，皮肤，肌腱，。骨关节，血管及神经损伤状况等9个因素的客观评分，进行多因素两类判别分析，并将判别方程前瞻性应用于26个断指，诊断其再植适应证。结果 判别方程为，Z＝0．24x1 3.49x2 1.72x3 1.42x4 4.96x5 1.22x6 1.39x7 5.38x8 2.08x9，判别临界值Z^*＝57．7,经方差分析，所建立程有高度显著意义（P＜0．01），26例断指前瞻性应用，诊断敏感性为94．7％，结论 本方程可用于诊断断指再植的适应证。

Quantitative diagnosis to indication of severed finger replantation

AIM To establish the quantitative diagnostic method for selecting indication of severed finger replantation. METHODS Records of 40 failed finger and 40 salvage fingers were retrospectively studied by injury score ruler,according to patients' age, mechanism of injury, prehospital duration, finger preservation as well as injury degree of skin, tendon, bone, vessel, nerve, the discriminant function was made in computer. RESULTS Discriminant function: Z= 0.24 x 1+3.49x 2+1.72x 3+1.42x 4+4.96x 5+1.22x 6+ 1.39 x 7+5.38x 8+2.08x 9, Z * =57.7. Blood vessel injury was the most important factor involved in the score, followed by the skin condition and injury mechanism. The discriminant method with clinical prospesstive application in 26 patients showed accuracy and promptness. CONCLUSION Discriminant method is of important value in diagnosing the severed finger suitable for replantation or amputation.
