Y型钢板和双钢板治疗成人肱骨髁间骨折疗效对比研究

目的探讨Y型钢板和双钢板治疗成人肱骨髁间骨折疗效差别。方法选取我院2005年1月至2010年12月收治的成人肱骨髁间骨折患者58例,患者根据治疗方式的不同分为Y型钢板组和双钢板组,每组29例,术后采用改良cassebaum肘关节评分系统评定,结果行统计学对比。结果 Y型钢板组肱三头肌舌型肌瓣入路1例患者没有骨性愈合,其他患者均达到骨性愈合。患者术后无迟发性尺神经炎、无内固定物松动及断裂等发生,无异位骨化。两组患者对比均显示采用尺骨鹰嘴截骨入路患者优良率显著高于采用肱三头肌舌型肌瓣入路者,结果对比差异有统计学意义,P〈0.05。结论尺骨鹰嘴截骨入路的双钢板治疗的肱骨髁间骨折患者的预后佳,并发症少,值得临床推广。

Comparative study on curative effects of Y style plate and dual plate in treatment of adult humeral intercondylar fracture

Objective To discuss the differences in curative effects between Y style plate and dual plate in treatment of adult humeral intercondylar fracture.Methods Totally 58 cases of adult humeral intercondylar fracture in our hospital from January 2005 to December 2010 were selected and divided into Y style plate group（n=29） and dual plate group（n=29） according to the different treating methods.Modified cassebaum elbow rating system was applied postoperatively and the results were analyzed statistically.Results All cases in Y style plate group received bony union except one case through triceps tongue-shaped flap approach.There was no inflameation of ulnar nerve,fixator loosening and rupture,ectopic ossification.The excellence rate in patients underwent approach of osteotomy olecranon with double-plate internal fixation was significantly higher that through triceps tongue-shaped flap approach,with satatiscal differences,P0.05.ConclusionApproach of osteotomy olecranon with double-plate internal fixation has better prognosis,fewer complications,therefore is worth promoting.
