肌电生物反馈电刺激对偏瘫踝背屈障碍的康复效果

摘    要:目的观察肌电生物反馈联合踝关节功能强化训练治疗偏瘫患者踝背屈障碍的疗效。方法脑卒中偏瘫患者60 例分为治疗组和对照组各30 例。两组均给予常规康复治疗和踝关节强化训练,治疗组在此基础上给予肌电生物反馈电刺激。两组于治疗前和治疗8 周后进行简式Fugl-Meyer 运动功能、Fugl-Meyer 平衡功能、踝关节主动活动范围评定,测定踝关节最大背屈时胫前肌等长收缩的肌电积分值(iEMG)。结果治疗后,两组运动功能、平衡功能、踝关节活动范围及iEMG 较治疗前均有明显提高(P<0.01),治疗组各项指标均优于对照组(P<0.05)。结论肌电生物反馈电刺激能促进偏瘫患者踝关节背屈功能改善。

Abstract:Objective To observe the effect of surface electromyogram biofeedback electrostimulation on ankle dorsiflexion in stroke hemiplegics.Methods 60 stroke patients were devided into treatment group(n=30) and control group(n=30).All the patients received routine rehabilitation and ankle intensive training.The treatment group received surface electromyogram biofeedback electrostimulation in addition.They were assessed with Simple Fugl-Meyer Assessment of motor and balance,active range of movement of ankle;and the integrated electromyogram(iEMG) of isometric contraction of anterior tibialis under maximum ankle dorsiflexion were determined before and after treatment.Results The scores of motor and balance,active range of movement of ankle,and the iEMG improved in both groups after treatment(P<0.01),but improved more in the treatment group than in the control group(P<0.05).Conclusion Surface electromyogram biofeedback electrostimulation can significantly improve ankle dorsiflexion in patients with hemiplegia.
