无骨折脱位型急性颈脊髓损伤的疗效观察

目的探讨无骨折脱位型急性颈脊髓损伤治疗方法的选择。方法回顾性分析非手术与手术治疗的42例无骨折脱位型急性颈脊髓损伤患者的疗效。分非于术组及手术组进行分析，观察各组手术治疗前后及随访时JJOA评分变化。结果所有病例获随访，JOA评分平均较术前改善，手术病例未发生血管、神经损伤等严重并发症。结论对无骨折脱位刑急性颈脊髓损伤应早期手术治疗，根据病理基础及受伤机理选择手术治疗，可以状得良好的临床疗效。

Choice of the treatments of acute cervical spinal cord injury without fracture and dislocation.

Objective To explore how to choose the methods of acute cervical spinal cord injury without fracture and dislocation. Method To analyze retrospectively the curative effect of 42 patients presenting with with non-surgical or surgical treatments, two groups were separately observed includig non-surgical group and surgical group, Each patient＇ s JOA score before and after operation was reviewed. Results Follow-up data were obtained in all patients. The JOA score improved after operation. No severe vascu neurological complication occurred in surgical group. Conclusion Patients should get early surgery for treatment of acute cervical spinal cord injury without fracture and dislocation. Surgical treatment of acute cervical spinal cord injury without fracture and dislocaotion can achive an efficient and safe clinical outcome, while reliable selection of surgical procedure on carfully considering the patient＇ s status before operation.
