18例异位胰腺的诊断与治疗分析

目的：探讨异位胰腺的临床特征、诊断方法及处理原则。方法对我院2000-2012年经手术切除病理确诊的18例异位胰腺患者的临床资料进行回顾性分析。结果胰腺异位至其它器官的患者例数分别为：胃部6例、空肠4例、十二指肠3例、胆囊2例、胆总管2例、纵膈1例。术前有2例确诊异位胰腺，7例误诊，9例漏诊。上述病例均行手术治疗并手术切除物样本病理切片证实，未出现术后并发症。结论异位胰腺的临床表现无特异性，实验室及影像学检查易出现漏诊和误诊。发现异位胰腺，无论有无临床症状，建议采取早期手术治疗。

Diagnosis and Treatment of Ectopic Pancreas in 18 Cases

Objective To discuss the clinical features, diagnosis and treatment of ectopic pancreas. Methods A retrospective analy-sis was made to the clinical data of 18 cases with ectopic pancreas confirmed by pathological study after surgical resection in the period from 2000 to 2012. Results The ectopic pancreas was located in stomach in 6 cases, in jejunum in 4 cases, in duodenum in 3 cases, in gallbladder in 2 cases, in common bile duct in 2 cases and in mediastinum in 1 case;before surgery, confirmed diagnosis of ectopic pan-creas was made in 2 cases while there were 7 cases of misdiagnosis and 9 cases of missed diagnosis;all the cases underwent surgery and pathological study was made to the samples of surgery resection for confirmation;no complications occurred. Conclusions The clinical manifestations of ectopic pancreas are non-specific, missed diagnosis and misdiagnosis may be made only with the lab test and imaging examination;early surgery should be performed when ectopic pancreas is considered even without clinical symptoms.
