PCR法检测肺炎支原体的临床应用及结果分析

摘    要:目的探讨PCR法检测肺炎支原体(Mycoplasma pneumoniae,Mp)的临床应用价值。方法选择213例社区获得性肺炎(community-acquired pneumonia,CAP)患者中临床证实的62例肺炎支原体感染者的咽拭子、血清标本,分别经肺炎支原体核酸PCR(Polymerase chain reaction,PCR)扩增和血清学检测(IgM)、肺炎支原体培养,采用四格表χ^2检验比较三种方法的检测结果。结果三种方法检测肺炎支原体(Mycoplasma Pneumoniae,MP)的阳性率分别为87.1%、72.6%、45.2%,其中PCR法与血清法比较无统计学差异(P〉0.05);与培养法比较有统计学差异(P〈0.01);血清法与培养法比较有统计学差异(P〈0.01)。结论PCR法检测肺炎支原体具有比培养法、血清法更高的敏感性和特异性,特别是对于早期的感染,对于临床早期的诊断和治疗具有重要意义。

Abstract:Objective To evaluate the PCR detection of Mycoplasma pneumoniae(Mp)value in clinical application.Methods 213 cases of community-acquired pneumonia(CAP)in patients with clinical confirmed 62 cases of Mycoplasma pneumoniae infection in the double throat swab,serum samples,respectively by PCR(Polymerase chain reaction)amplification of Mycoplasma pneumoniae nucleic acid and serological testing(IgM),culture,the use of fourfold table χ^2 test comparing the test results in three ways.Results The three methods of detection of Mp-positive rates were 87.1%,72.6%,45.2%,and serum-PCR method with no significant difference(P〉0.05);and culture there was a significant difference(P〈0.01);serum law and culture had more significant difference(P〈0.01).Conclusion PCR detection of Mycoplasma pneumoniae than culture,higher serum method sensitivity and specificity,especially for early infection,early clinical diagnosis and treatment is important.
