IVF—ET周期中第三天移植胚胎的原核期形态学研究

摘    要:目的为选择更有发育潜能的胚胎,对第三天移植周期中种植成功并分娩的胚胎与未种植的胚胎从原核期形态学方面进行比较。方法选择2006年1~12月在我所接受IVF-ET助孕的433个周期进行回顾分析,分为1组(植入组),85个周期;2组(未植入组),348个周期。比较两组所有移植胚胎中不同原核期评分的分布和胚胎发育情况。结果1.1组168个胚胎,2组719个胚胎,两组原核分布百分比相比较无统计学差异(P〉0.05)。2.第二天原核评分为Z1的胚胎中,1组碎片量明显小于2组(P〈0.05);评分为Z2的胚胎中,1组比2组胚胎评级更好(P〈0.05)。3.第三天原核评分为Z2的胚胎中,1组比2组第三天细胞数更多、胚胎评级更好(P〈0.05);原核评分为Z3的胚胎中,1组比2组细胞数更多(P〈0.05)。结论目前的原核评分系统并不能很好地预测胚胎的种植潜能,以卵裂期评分为主结合原核评分的胚胎选择方法在目前是适宜的。

The pronuclear morphology study of D3 embryos transferred in IVF - ET cycles

Abstract:Objective : For chose the embryos have better development potential, the Pronuclear (PN) morphology scores were compared between embryos implanted successfully or not. Methods : 433 fresh IVF - ET cycles performed in our institute from January to December 2006 were analyzed retrospectively. According to the embryos implanted successfully or not, all cycles were divided to 2 groups. Embryos transferred of group 1 (85 cycles) had implanted and delivered successfully. 348 cycles of group 2 had not. Embryos transferred of 2 groups were compared according to the morphology scores of PN stage. Results : 1. The distribution percentages of PN scores had no difference in group 1 (168 embryos ) and group 2 (719 embryos). 2. The day2 fragments of Z1 in group 1 were fewer than group 2. The day2 scores of Z2 in group 1 were better than group 2. 3. Z2 of group 1 had more day3 cells and better embryo scores when compared with group 2. Z3 of group 1 had more day3 ceils than group 2. Conclusion: The PN score system we used could not predict the implantation potential preciously. The chose criteria of cleavage morphology priority were fit for the clinic use to choose embryo when combined with the PN scores.
