三角形子宫次全切术式探讨

目的探讨三角形子宫次全切除术的最佳方法和效果。方法选择2007年1月～2008年12月因子宫肌瘤、功能性子宫出血需行子宫切除的住院患者25例为观察组;另随机选择同期手术患者27例为对照组。结果观察组手术时间、出血量、术后排气时间和下床活动时间与对照组比较,差异无统计学意义(P0.05);观察组术后月经情况、性生活质量、下腹不适、更年期症状等方面与对照组比较,差异有统计学意义(P0.01)。结论三角形子宫次全切除术,不切断子宫血管及周围韧带,既去除了子宫良性病变又保留了完整的子宫动脉上行支,使卵巢功能不受影响;同时保留了子宫的形态、保持了盆腔的完整性,不影响盆腔的正常解剖结构,未改变盆腔的支持力,因而术后疼痛较轻,无下腹坠胀感和盆腔空虚感,并可防止内脏脱垂,值得推广。

Exploration of Triangle Subtotal Hysterectomy

Objective To explore the optimum method and effect of triangle subtotal hysterectomy.Methods 25 inpatients required hysterectomy due to uterine fibroids and dysfunctional uterine bleeding were selected as observation group from January 2007 to December 2008.Additionally,27 cases of operation in the same period were optioned randomly.Results There was no statistical significant difference in operation time,bleeding volume,gas-discharge time and time of leave-bed movement after operation in comparison between observation group and control group（P0.05）.In observation group,the statistical significant differences presented in postoperative menstrual situation,sexual quality,the lower abdominal discomfort and menopause symptoms as compared with control group（P0.01）.Conclusion The triangle subtotal hysterectomy does not cut off uterine vessel and peripheral ligaments,which not long removes benign uterine disorder,remains the up-going branch of uterine artery and does not impact on ovary function,but also remains uterine morphology,keeps pelvic integrity and does not impact on the normal anatomic structure pelvic cavity and change the supporting structure of pelvic cavity.As a result,the post-operative pain is alleviated,no longer with bearing-down sensation in the lower abdomen and empty feeling in pelvic cavity.Moreover,the visceral prolapse can be prevented.Hence,this operation deserves to be promoted.
