上尿路结石ESWL后中药协同排石的临床观察

目的　探讨上尿路结石体外冲击波碎石 (ESWL)后理想的中药配合治疗 ,以提高结石患者的治愈率、减少ESWL后并发症的发生与结石残留 ,有效的降低结石复发及减少药物副作用的发生。方法　对 52 80例上尿路结石患者ESWL后随机分为两组 ,服用肾石通冲剂组 31 70例 ,服用排石汤组 2 1 1 0例。结果　 (1 )治愈率 :肾石通冲剂组为 90 .73 % ,排石汤组为 79.48% ;(2 )石街形成率 :肾石通冲剂组为 1 .58% ,排石汤组为 4 .50 % ;(3)结石残留 :肾石通冲剂组为 7.2 6 % ,排石汤组为 1 5 .40 % ;(4)副作用 :出现恶心、呕吐与食欲减退者肾石通冲剂组为1 4 .98% ,排石汤组为 50 .0 5 % (少数患者还出现腹泻症状 ) ;(5)结石排出时间 :肾石通冲剂组平均为 1 .5天 ,排石汤组平均为 3 .7天。结论　在ESWL后配合服用肾石通冲剂能提高结石的治愈率 ,减少并发症与结石残留 ,缩短结石排出时间 ,且副作用少 ,与排石汤相比治疗效果更为满意

Discussion on the medication of calculus of upper urinary tract after ESWL

Objectives To discuss the ideal matching medication of calculus of upper urinary tract with traditional Chinese medicine after external shock wave lithotrity (ESWL) in order to improve cure rate of calculus patients, decrease post-ESWL complications and residual calculus, and reduce relapse of calculus and occurrence of side-effects.Methods 5280 cases of calculus of upper urinary tract were randomly divided into two groups.One group of 3170 cases were treated with Shenshitong Granules and the other group of 2110 cases with Paishitang.Results The cure rate of Shenshitong group was 90.73% and that of Paishitang group was 79.48%. The rate of formation of calculus lining was 1.58% in Shenshitong group and 4.50% in Paishitang group. Residual calculus occurred in 7.26% of Shenshitong group and in 15.40% of Paishitang group. Side-effects such as nausea, vomiting and anorexia were found in 14.98% of Shenshitong group and in 50.05% of Paishitang group (a few cases also had symptoms of diarrhea). The average discharging time of calculus was 1.5 days in Shenshitong group and 3.7 days in Paishitang group.Conclusions The medication of Shenshitong Power after ESWL could improve the cure rate of calculus, reduce complications and residual calculus, and advance the discharging time of calculus. It yielded more satisfactory therapeutic effects than Paishitang with fewer side-effects.
