三种手术方法治疗兔Perthes病股骨头的VEGF表达

目的]采用3种不同的手术方法(髓芯减压、滑膜切除和自体松质骨移植)治疗Perthes病模型,通过观察并比较VGEF表达的变化对其治疗效果作评价.方法]36只3个月龄新西兰大白兔随机平均分为A、B、C、D4组.手术切断左侧圆韧带和股骨头支持带血供,建立兔Perthes病模型.模型制作后2周对B、C、D组分别采用髓芯减压、滑膜切除、自体松质骨移植方法加以治疗.结果]大体观察:A、B、C三组均在8周发现股骨头较健侧变小,部分标本可见塌陷.D组8周股骨头软骨下骨未见明显塌陷.组织学观察:A、B、C三组8周才可见到部分修复.D组4周即可见纤维肉芽组织增生,部分新骨形成.免疫组织化学:B、C组各时间点VEGF免疫反应性(immunoreactivity,VEGF-IR)与A组无明显差异.D组各时间点增殖区与肥大区VEGF阳性细胞率高于其他各组,相比有统计学意义(P＜0.05,α=0.05).结论]自体松质骨移植手术治疗Perthes病模型中,VEGF的表达明显高于其他两组,组织病理上也发现本组股骨头骺软骨较其他两手术治疗组修复明显.
