经阴道彩色多普勒超声在早期诊断输卵管妊娠中的价值

目的 通过研究输卵管妊娠和正常妊娠子宫及双附件经阴道彩色多普勒超声影像学特点,探讨其对输卵管妊娠的早期诊断价值。方法 对30例输卵管妊娠患者行经阴道彩色多普勒超声检查,观察子宫动脉、子宫螺旋动脉、附件区包块内及周边血流信号,并测定血流频谱的阻力指数(RI)。同期采用同样方法检查30例正常早期妊娠的患者。结果 通过经阴道彩色多普勒超声对可疑输卵管妊娠进行诊断,结合子宫动脉血流平均RI值、内膜螺旋动脉血流显示率及附件区包块有无滋养层动脉血流这3项指标检测结果,以手术治疗病理结果为标准,最后诊断输卵管妊娠的准确率为93.4%。结论 经阴道彩色多普勒超声对子宫附件相关部位血流动力学进行定性或定量检测,为输卵管妊娠的早期诊断及鉴别诊断提供更多有价值的信息,具有重要的临床价值。

Application value of transvaginal color Doppler sonography in early diagnosis of tubal pregnancy

Objective To study the imaging features of application pregnancy and normal pregnancy by applying transvaginal color Doppler sonography (TVS-CDFI), and discuss the early diagnostic value in application pregnancy. Methods A total of 30 cases of patients with application pregnancy would be examined by TVS-CDFI. The uterine artery, spiral artery, the blood flow signal within and surrounding accessory masses were observed by TVS-CDFI, and the resistance index of blood flow frequency spectrum was measured. The 30 cases of normal pregnancy were also studied in the same term. Results Suspected application pregnancy could be diagnosed by TVS-CDFI, and combined with average uterine artery RI, the detection rate of endometrial spiral artery blood flow and the presence of trophoblastic artery flow. The accuracy rate of diagnostic application pregnancy turned out to be 93.4%, based on the pathological results. Conclusion TVS-CDFI can qualitatively and quantitatively detect blood flow. TVS-CDFI has important clinic value in providing valuable information in diagnosing application pregnancy at early stage.
