乳腺癌超声特征及其与ER、PR、C-erbB-2表达关系及临床意义

摘    要:目的初步探讨乳腺癌超声特征与免疫组织化学因子雌激素受体(ER)、孕激素受体(PR)、原癌基因(C-erbB-2)表达的关系,为临床乳腺癌治疗及预后判断提供更多的信息和科学依据。方法应用二维超声、彩色多普勒超声研究85例乳腺肿块的超声改变,总结乳腺癌的超声特征;将其与免疫组织化学方法检测的ER、PR及C-erbB-2因子表达结果相对照,分析乳腺癌超声特征与ER、PR及C-erbB-2因子表达的关系。结果乳腺癌与乳腺良性肿块的二维超声图像特征均有显著性差异(P〈0.01)。同时,有毛刺、后方衰减的乳腺癌有更高的ER阳性表达率(P〈0.05);腋窝淋巴结转移、彩色多普勒血流丰富的乳腺癌有更高的C-erbB-2阳性表达率(P〈0.05)。结论二维超声及彩色多普勒超声的特征性改变,在乳腺癌诊断中具有重要意义;乳腺癌超声特征与ER、PR、C-erbB-2的表达密切相关。

Abstract:【Objective】Toinvestigate the correlation between ultrasonic characteristics and the immunohistochemical expression of estrogen receptor(ER),progesterone receptor(PR) and C-erbB-2 in breast cancer,provide more information and scientific evidence for the therapy and evaluation of the prognosis.【Methods】The ultrasonic characteristics were compared with the results of immunohistochemical ER,PR,C-erbB-2 expression and were analyzed the correlation between ultrasonic characteristics and the three factors in breast cancer.【Results】The positive expression rate of ER was higher in breast cancer with spiny edge and posterior echo attenuation than those with no spiny edge and no posterior echo attenuation,there was difference between them(P<0.05);the positive expression rate of C-erbB-2 was higher in breast cancer with axillary lymph nodes metastasis and plentiful blood stream than those with no axillary lymph nodes metastasis and no plentiful blood stream(P<0.05).【Conclusion】Ultrasonic characteristic changes of Two-dimensional Ultrasound and Color Doppler Flow Imaging have an important significance in breast cancer diagnosis.The ultrasonic characterstics of breast cancer are closely correlated with the expression of ER,PR and c-erbB-2.
