早期糖尿病肾病患者血清Cys-C、β2-MG、BUN、SCr临床观察与研究

摘    要:目的 探索检测血清胱抑素C (Cys-C),与β2微球蛋白(β2-MG)、血清肌酐(SCr)、尿素(BUN)对糖尿病肾病患者早期诊断的价值.方法 应用颗粒增强免疫比浊法测定101例健康体检者(对照组)和112例糖尿病患者(糖尿病肾病组)的血清胱抑素C、β2微球蛋白;酶法测定血清肌酐及尿素浓度.结果 早期糖尿病肾病组Cys-C(1.251±0.43)、β2-MG (3.49±0.48)、SCr(74.2±21.2)、BUN (5.12±1.91)均较对照组明显增高,差异有统计学意义(P<0.01);早期糖尿病肾病组Cys-C、β2-MG和BUN、SCr阳性率分别为80.8%、88.4%、31.2%、33.0%;Cys-C与β2-MG的相关系数r =0.99589,差异有统计学意义(P<0.01).结论 胱抑素C是一个能较好反映肾小球滤过率的指标,其评价糖尿病患者肾功能状况的特异性好于β2微球蛋白,灵敏度高于SCr、BUN.

Abstract:Objective To explore the clinical effect of detecting the contents of serum Cys-C,β2-MG, SCr and BUN on the early diagnosis of diabetic nephropathy patients. Methods The particle reinforced immunoturbidimetry was used to determine serum Cys-C and β2-MG of 101 healthy people and 112 cases of diabetes patients, and the enzyme method was used to determine SCr and BUN. Results Compared with detected results of healthy control group, the contents of Cys-C, β2-MG, SC and BUN were1. 251±0.43, 3. 49±0.48, 74.2±21.2 and 5.12±1.91 respectively in the early diabetic nephropathy group ( P〈0.01). In the early diabetic nephropathy group, the positive rates of Cys-C, β2-MG, BUN and SCr were 80.8%,88. 4%, 31. 20% and 33. 0% respectively and the correlation coefficient of Cys-C and β2-MG was r = 0. 99589( P 〈0.01). Conclusion Serum Cys-C concentration can effectively indicate glomerular filtration rate and can be used to better evaluate diabetes patients' renal functions for its higher specificity than
