摘    要:目的 研究微移电位 (inching)对嵌压性神经病的定位诊断价值。方法 对 5 8例确诊的累及运动功能的嵌压性单神经病患者进行受累神经干上间隔 2cm的距离逐点测试 ,并同时给予常规神经电图检查。结果  5 8例 76条神经中 5 1例 ( 88% ) 6 5条神经 ( 85 % )跨嵌压段一项或几项指标异常 ,其中波幅明显衰减 6 0条 ( 79% ) ,传导减慢 4 0条 ( 5 3% ) ,潜伏期延长 4 2条 ( 5 5 % ) ,波形显著变化 5 5条 ( 72 % ) ,波宽显著变化 5 4条 ( 71% )。阳性率最高的表现是波幅降低。与常规神经电图总的阳性率为 6 0 % ( 4 6条 )作比较 ,逐点测试阳性率明显提高 (P <0 .0 1)。结论 逐点测试对嵌压性单神经病有重要的定位诊断价值 ,其中跨嵌压段波幅显著衰减比传导速度的减慢有更高的阳性率

Abstract:Objective To study the location diagnostic value of inching test in compressive neuropathy.Methods The inching test was performed in 58 patients with compressive simple neuropathy with motor function involved,and the routine EMG was done.Results One or more indexes of inching test were abnormal in 65 nerves (85%),among 76 ones detected.The decrease of amplitude,of conduction velocity and prolongation of latency were noted in 60 nerves (79%),40 ones (53%) and 42 (55%) respectively.The significant changes of wave form and wave duration was found in 55 nerves (72%) and 54 (71%) respectively.The total positivity rate of the electroneurogram was 61%.Conclusion The inching test might have an important role in the location diagnosis of compressive simple neuropathy.The significant decrement of amplitude might have higher posstivity rate as compared with that of the decreace of the conduction velocity.The degree of amplitude decrease might have a guide role in the choice of treatment method.
