改良青光眼小梁切除滤过术及其临床应用

目的对青光眼小梁切除滤过术进行改良，以降低青光眼小梁切除滤过术后房水引流不畅及斑痕形成发生率。方法利用自体巩膜条作为引流物，在常规小梁切除术后的巩膜瓣后，再做一板层巩膜条，植入巩膜瓣下．引流房水。结果治疗38例（50只眼），术后早期前房形成良好，眼压正常；随访2年，1只眼滤过泡消失（手术失败），49只眼维持功能滤过泡，眼压正常。结论自体巩膜作为引流物能长期保持滤过功能及防止并发症。

Clinical application of modified glaucma trabeculetomy

Objective:To prevent inadequate drainage of aqueous hunor and the postoperative cicatrice after glaucoma filtering surgery.Methods:Turning scleral flap of selfbody was performed over drainage surgery.Results:The anterior chambers in 50 eyes were well-formed by postoperative observation and the intraocular pressures were normal.2 year later,only 1 filtration bleb disappeared,filtration blebs in anther 49 eyes maintained functional conjunctival blebs of filtration,and intraocular pressures remained normal.Conclusion:This study suggests that the turning scleral flap of selfbody over drainage surgery is helpful to maintain filtering function for a long time and to prevent complcation after filtering surgery.
