肝癌特异性AFP亚型测定方法及诊断价值研究

目的：研究肝癌特异性甲胎蛋白(AFP)亚型(Hs—AFP)对原发性肝细胞癌(PHc)的诊断和鉴别诊断价值。方法：建立聚丙烯酰胺凝胶电泳和Western blot分离检测血清HS—AFP的方法，对82例PHC和95例良性肝病患者HS～AFP进行分离检测，分析HS—AF甲与血清AFP浓度、肿瘤大小及门静脉转移的关系。结果；肝癌患者HS—AFP阳性者为74．4％，明显高于良性肝病(肝硬化、慢性肝炎和急性肝炎的阳性率分别为9．1％、7．3％和10．0％)。HS—AFP与血清AFP浓度和肿瘤大小有一定相关性，而与有无门静脉转移关系不大。结论：HS—AFP可提高AFP阴性PHC和早期PHC的诊断敏感性．尤其对良恶性肝病所致的AFP升高有较高鉴别诊断价值。

Clinical value of hepatoma-specific alpha-fetoprotein in the diagnosis of primary hepatocellular carcinoma

Objective:To study the clinical value of hepatoma-specific alpha-fetoprotein(HS-AFP) in the diagnosis and differential diagnosis of primary hepatocellular carcinoma(PHC).Methods:HS-AFP was separated and determined with polyacrylamide electrophoresis combined with Western blot in 82 cases with PHC and 95 cases with benign liver diseases.The correlations between positive rates of HS-AFP and serum AFP concentration,tumor size as well as portal vein metastasis were analyzed.Results:The positive rate of HS-AFP in PHC was 74.4%,which was significantly higher than that in benign liver diseases(9.1%,7.3% and 10.0% for liver cirrhosis,chronic hepatitis and acute hepatitis respectively).HS-AFP is correlated with serum AFP concentration and tumor size to some extent,but not with portal vein metastasis.Conclusions:The diagnostic sensitivity can be increased by HS-AFP in PHC cases with negative AFP or at early stage.And HS-AFP is especially useful in distinguishing high AFP due to PHC from benign liver diseases.
